I accept, declare, and undertake that all my personal data — including special categories of personal data and personal health information — provided verbally, in writing, and/or electronically to HF media may be processed by HF media for purposes such as candidate selection, execution of employment contracts, fulfillment of obligations arising from such contracts, and provision and planning of healthcare services. Furthermore, I consent to the sharing of such data with relevant persons and organizations where legally required, within the framework of applicable legislation, including the Law No. 6698 and the Regulation on Processing and Protection of the Privacy of Personal Health Data.
“Personal data” refers to any information relating to an identified or identifiable natural person.
“Special categories of personal data” include information about a person’s race, ethnic origin, political opinion, philosophical belief, religion, sect or other beliefs, appearance and clothing, association, foundation or trade union membership, health, sexual life and orientation, criminal conviction, security measures, and biometric, medical, or genetic characteristics.
The following personal and special category data, which clearly belong to an identified or identifiable natural person, may be processed by HF media automatically or non-automatically as part of a data recording system:
Identity Information: Data such as name-surname, Turkish ID number, nationality, parents’ names, place and date of birth, gender, and information contained in ID cards, driver’s licenses, or passports; as well as tax number, social security number, signature, vehicle plate number, etc.
Contact Information: Data such as phone number, address, e-mail address, fax number, and IP address.
Security Information: Data obtained during physical entry into or presence within company premises, including CCTV recordings, fingerprint records, and other security logs.
Visual and Audio Data: Photographs and video recordings (excluding those collected for physical security purposes), and data contained in personal documents.
Sensitive Data: Data defined in Article 6 of the Law, including health data (e.g., blood type), biometric, and similar sensitive personal information.
Request and Complaint Information: Data related to any requests or complaints submitted to HF media.
